Fostering Psychological Safety and a Just Safety Culture

The presentation will include:

  • A review of the Radonda Vaught prosecution and conviction (the criminalization of medication error)
  • An overview of the importance of creating a psychologically safe environment and fostering a just culture
  • A description of the critical elements of a system-based event review and the importance of asking "serial Whys"
  • An introduction to the three behaviors associated with safety events (normal error, at risk behavior, and reckless/egregious behavior)
  • An interactive application of the Just Culture Algorithm using case studies
